Services at The Community Free Clinic

The Community Free Clinic provides free healthcare services for eligible uninsured residents living in Cabarrus County. Our clinic in Concord, NC offers primary care, chronic condition support, diagnostic testing, medication assistance, lifestyle medicine, and coordinated referrals through HealthNet Cabarrus.

All services are provided at no charge for eligible patients. If a service is not offered onsite, our team helps coordinate next steps through trusted community partners.

Primary Care and Ongoing Support

Primary Care Visits

Primary care includes routine visits, evaluation of new symptoms, preventive screenings, and follow up care. Patients receive ongoing support, not one time treatment.

Chronic Condition Management

We help patients manage long term conditions such as high blood pressure, diabetes, asthma, and other chronic illnesses. Care may include monitoring, medication support, and lifestyle guidance.

Specialty Care and Coordinated Referrals

Specialty Clinics and Visiting Provider

Specialty support is provided through scheduled clinics and visiting providers, based on availability. Services may include optometry, in-office procedures, women’s care and other specialty care when available through onsite clinical volunteers.

HealthNet Cabarrus Referral Coordination

HealthNet Cabarrus helps connect eligible patients to participating providers across Cabarrus County through a coordinated referral process. This coordination matters because specialty services often require documentation, scheduling, and follow up. Our team helps patients navigate those steps.

Diagnostic Testing and Lab Services

Lab Work and Routine Testing

Diagnostic testing helps guide treatment plans and monitor chronic conditions. Testing is ordered based on clinical need and completed through clinic processes and lab partnerships.

Common Testing and Screening Examples

Examples may include blood pressure, blood glucose, urinalysis, strep testing, pregnancy testing, EKG, retinal scanning, and other routine screenings as appropriate.

Medication Access Support

Medication access is a key part of chronic disease management. The clinic helps eligible patients access prescribed medications and understand how to take them safely.

NC MedAssist Partnership

The clinic partners with NC MedAssist, a nonprofit pharmacy, to help eligible patients receive medications. The MedAssist pharmacy application is a separate process, and clinic staff can assist patients with completion when needed.

Health Education and Coaching

We provide education and coaching to help patients understand diagnoses, follow treatment plans, and make practical changes that support long term health.

Behavioral Health and Support Services

Support services may include behavioral health support, nutrition counseling, diabetes education, and help connecting to local resources when appropriate.

Lifestyle Medicine and Food Pharmacy

Lifestyle Medicine

Lifestyle medicine supports long term health through practical education and coaching focused on daily habits that impact wellbeing.

Food Pharmacy

The Food Pharmacy is part of the lifestyle medicine program. Eligible patients may receive nutrition education and coaching and may be enrolled in cooking classes. Patients pick up food on a biweekly basis and have an opportunity to meet with clinic staff during pickup.

The Clinic provides free food pantry bags to patients.

Dental Support

Dental support is limited and varies based on clinic capacity. When additional dental services are needed, patients may be referred to community partners through coordinated referral support.
