About The Community Free Clinic

The Community Free Clinic is a nonprofit healthcare organization serving uninsured residents in Cabarrus County, North Carolina. Since 1994, the clinic has helped residents access consistent medical care when they cannot afford insurance.

Located in Concord, NC, the clinic provides free primary care, chronic disease management, medication support, diagnostic testing, and coordinated specialty referrals through HealthNet Cabarrus.

What We Do

We provide free, coordinated medical care for eligible uninsured adults in Cabarrus County. Our care model is built around ongoing primary care, clear enrollment processes, and coordinated referrals when specialty services are needed.

Care is provided at no charge for eligible patients. Services are based on medical need and clinic capacity.

Who We Serve

The clinic serves uninsured adults who live in Cabarrus County and meet income guidelines up to 300% of the Federal Poverty Level.

Patients may not qualify for Medicare, Medicaid, or VA benefits and rely on the clinic for a consistent point of care and referral coordination.

Our History

The Community Free Clinic was established in 1994 in response to the growing number of uninsured adults without access to medical care in Cabarrus County.

Over time, the clinic expanded services and strengthened partnerships across the county to improve access to both primary care and specialty services through coordinated referrals.

HealthNet Cabarrus

The Community Free Clinic serves as the enrollment hub for HealthNet Cabarrus, a coordinated network that connects eligible patients to participating providers throughout Cabarrus County.

Enrolled patients receive the Pink Card and may access specialty services through a structured referral process when services are not available onsite.

Impact You Can Measure

The clinic focuses on measurable health outcomes. Last year, 74% of individuals with hypertension controlled their blood pressure within the year, and 66% of patients with diabetes maintained controlled A1C levels over an extended period.

During the 2024 to 2025 fiscal year, clinic services saved the hospital system $3.5 million. For every dollar spent, the clinic provided $13.22 in services.
