Patient Enrollment and Re-Enrollment

If you need healthcare and do not have insurance, The Community Free Clinic may be able to help.

This page will help you start a new application or complete your yearly re-enrollment. You can apply online, print forms, or visit the clinic for help. Enrollment specialists are available Monday through Thursday, 8:30 AM to 5:00 PM, and help is available in English and Spanish.

Requirements

Who Can Qualify

You may qualify if you:

  • Live in Cabarrus County

  • Do not have health insurance

  • Meet income guidelines

  • Do not qualify for Medicare, Medicaid, or VA benefits

What You Need

Have these ready before you apply.

Proof of Address

  • Driver license or ID with address

  • Utility bill or letter (last 30 days)

  • Lease agreement

Proof of Income

  • Pay stubs (last 30 days)

  • Benefit letters (SSI, disability, unemployment)

  • Employer verification

Medication Help

If you need help paying for prescriptions, you must complete a second form.

This program is called MedAssist. It helps you get long-term medications at no cost.
