Patient Enrollment and Re-Enrollment
If you need healthcare and do not have insurance, The Community Free Clinic may be able to help.
This page will help you start a new application or complete your yearly re-enrollment. You can apply online, print forms, or visit the clinic for help. Enrollment specialists are available Monday through Thursday, 8:30 AM to 5:00 PM, and help is available in English and Spanish.

Requirements
Who Can Qualify
You may qualify if you:
Live in Cabarrus County
Do not have health insurance
Meet income guidelines
Do not qualify for Medicare, Medicaid, or VA benefits
What You Need
Have these ready before you apply.
Proof of Address
Driver license or ID with address
Utility bill or letter (last 30 days)
Lease agreement
Proof of Income
Pay stubs (last 30 days)
Benefit letters (SSI, disability, unemployment)
Employer verification
Downloadable Application Forms
You can download the application form, complete it, and upload it when prompted. If you prefer, you may print and bring your application to Enrollment Services.
Medication Help
If you need help paying for prescriptions, you must complete a second form.
This program is called MedAssist. It helps you get long-term medications at no cost.
