Eligibility for Care

The Community Free Clinic provides free medical care for uninsured residents who live in Cabarrus County, North Carolina. To receive services, patients must meet residency, insurance, and income guidelines.

Review the requirements below before starting your application. Information submitted for enrollment is reviewed confidentially. If you have questions, contact Patient Services for assistance.

Basic Requirements

Cabarrus County Residency

You must live in Cabarrus County. Proof of residency is required during enrollment.

Uninsured Status

You must not have active health insurance coverage.

Income Guidelines

Household income must be at or below 300% of the Federal Poverty Level.

For reference, this level is approximately:

  • $43,740 per year for one adult

  • $90,000 per year for a family of four

Income guidelines are based on federal standards and may change annually.

Who May Not Qualify

You may not qualify for services if you have active health insurance or if you qualify for Medicare, Medicaid, or Veterans Affairs benefits.

Required Documentation

The following documents are required to complete enrollment:

Proof of Residency

  • North Carolina ID with current address
  • Utility bill
  • Lease agreement
  • Official mail with name and address

Proof of Income

  • Recent pay stubs
  • Benefit letters
  • Other documentation showing household income

Enrollment Process

Enrollment is required before receiving services. Applications are reviewed by the Enrollment Team to confirm eligibility and documentation.

Enrollment is required before an appointment to see a provider. The Patient Services team will confirm all information meets the three requirements of care.

Once approved, care is available for one calendar year. Re-enrollment is necessary annually. Once the Patient Services team receives all information, the team will schedule a first appointment.
